How to Create a Web3 Form

A web3 form allow creators get feedback from target audience

Follow the steps below to create a web3 Form
1.On your side menu, scroll down to Web3 Form and click on Overview
2.Click Create Form to create a web3 form
3.Fill the form with the appropriate information then click submit
Web3 Form
Title 
Your form title
Requirement type
The type of asset the respondent must have. eg solana coin
Select Coin
If your requirement type is Solana coin then you have to select the coin type. if your reqirement type is Web3 NfT then you will have to paste the NFT contract address where you have the select coin
Amount
The minimum amount of assets the respondent must have. Eg: 1 $Gary
Response Type
You can restrict the response time of respondents to one time or multiple times.
Status
Set status to active.

Click the form title to view responses.
